"Tooth wear" is the main factor to consider when making this decision.
If the teeth have become pointed, or if the belly or sides are worn, it's time for a replacement.
It is recommended to replace the front and rear sprockets and the chain all at the same time. If you only replace one of these parts, the worn parts that haven't been replaced will prevent the new part from performing optimally and lasting as long as it should. Considering the long-term, replacing all three at once is more cost-effective as they will last longer.
